A note on Hazard Reporting... The Worley SMS Safety Policy includes this statement:

All personnel are encouraged to help identify hazards, systemic errors, and ways of improving safety, and may do so confidentially if desired.  A confidential reporting system is in place for personnel to report hazards, accidents, incidents, and safety issues without fear of reprisal.

An aviation SMS must allow for confidential and anonymous reporting, and must allow anyone involved in the operation to report hazards, even if they are not Worley staff; e.g. clients or subcontractors.  Worley’s primary hazard reporting system does not meet these aviation-specific requirements; therefore, R3 and the Leadership Team have approved SMSPro as the hazard reporting system for aviation safety.
